Health is more than medical care itโ€™s coordination, access, and support. WNYILโ€™s Health Homes Care Coordination Services provide free, person-centered care for Medicaid recipients with chronic conditions, HIV/AIDS, or serious mental illness.

Health Homes are not a location, but a network of providers working together to improve health, reduce hospital visits, and connect people to services like housing, transportation, and behavioral health.

Each member is paired with a care manager to create a personalized care plan and navigate the healthcare system.

Alcohol Awareness Month is a time to raise understanding, reduce stigma, and remind people that recovery is possible.
The A2A (Addiction to Advocacy) and F2F (Family to Family) teams are available to adults in Erie and Niagara Counties who decide they are ready for recovery and a new life.

Individuals are matched with certified, trained Peer Advocatesโ€”people who are in recovery themselves or who have family members impacted by substance use disorders. Our Peers share lived experience, provide guidance, and walk alongside individuals as they explore their own path to recovery.

Our Peer Teams:
โ€ข Share recovery experiences while offering support, resources, and advocacy
โ€ข Help individuals navigate recovery options to prevent relapse and potential overdose
โ€ข Provide peer counseling, support, and education for family members and loved ones who want to help but may feel overwhelmed or frustrated.

Did you know that autism spectrum disorder is 4.3 times more prevalent in boys than it is in girls, according to the Centers for Disease Control?
This is likely because girls often go undiagnosed. They donโ€™t always fit the stereotypes people have about autism, and they may mask their symptoms better.

Today we recognize the strength, resilience, and courage of individuals living with bipolar disorder. Mental Health Peer Connection (MHPC), an organization run for and by people with behavioral health disabilities, provides person-centered advocacy, peer counseling, transitional services, independent living skill building, information and referral, and employment supports to help individuals thrive.

MHPC fosters meaningful connections to enhance the quality of life for residents of Erie County through programs at our satellite offices, Erie County Medical Center clinics, drug treatment courts, the Renewal Center, rehab facilities, re-entry programs, soup kitchens, homeless shelters, and public libraries.
