Since 1997, Lake Plains Community Care Network (LPCCN) has fostered collaboration among area health care providers to keep healthcare local and to improve local healthcare. LPCCN is a non-profit organization run by a board of local volunteers who are leaders in business, healthcare and government. The network promotes cooperation, the practice of preventative medicine, and access to quality care in Genesee, Orleans and Wyoming Counties.

You and your family have many low costs, quality health insurance options available through the Individual Marketplace.
Lake Plains Community Care Network will assist you to quickly compare health plan options and apply for assistance that could lower the cost of your health coverage. You may also qualify for health care coverage from Medicaid or Child Health Plus through the Marketplace. Anyone can apply by calling 585-345-6110 to talk to a Navigator.

Lake Plains Community Care Network has a new office. We are now located at 319 West Main Street, in the Crickler Building. Access is through the front door, turn right and we are the first office on the right.

We continue to limited office hours. Please call before coming to the office. 585-345-6110.

Julie Carasone, our Cultural Competence and Health Literacy Operations Specialist and Certified Trainer, just presented an Implicit Bias training for Genesee Justice. Training for these type of issues are so important in today’s times. We are proud that Genesee County officials are taking these issues seriously and welcomes these training.

Our very own Cultural Competence and Health Literacy Operations Specialist, Julie Carasone, did a presentation for Genesee County Management and the new County manager in regards to a very important topic, Implicit Bias. A topic that is being discussed in our community and across the nation.
It was met with requests for different agencies to have Julie come in and present to their staff. Genesee County is taking the wonderful steps in understanding the barriers that many in our most vulnerable communities have.

Incorporated in 1997, the mission of Lake Plains Community Care Network, Inc., (LPCCN) is to keep rural health care a local matter and to improve local health care. Its long term goals are to promote maximum access to local rural health services at the appropriate level of care through a locally-controlled multi-disciplinary community service network to achieve community accountability, a seamless continuum of services, improves community health and effective stewardship of limited rural services.
