📣We’re proud to share that Jen McIlrath, Chief Nursing Officer, represented Transitions at this week’s Illinois Hospice and Palliative Care Organization Regional Meeting, where she shared key strategies for strengthening hospice quality through effective HOPE tool implementation.

Here are the actionable practices Jen highlighted:

• Run daily operational reports to catch missed visits, documentation gaps, and schedule issues before they affect compliance or care delivery.

• Review HUV schedules closely to ensure all visits occur within CMS-required timeframes.

• Audit symptom-impact responses to ensure clinicians assign the correct visit type and intervention based on accurate scoring.

• Monitor iQIES submissions regularly to avoid rejections, late filings, or incomplete assessments.

It’s been a week full of celebration, gratitude, and connection! 💜

Bay City celebrated a special 100th birthday for one of our lovely patients. We marked Nacho Day to celebrate Ignite staff alongside a great community partner. And we joined in recognizing excellence in care at The Bronze Award Ceremony.

Each moment reflects what Transitions is all about, honoring life, supporting our teams, and celebrating the people who make our communities so special.
