The “second heart” in the calves refers to the function of the calf muscle pump, particularly the gastrocnemius and soleus muscles, in aiding venous blood return to the heart.

This system is especially crucial in the lower extremities, where blood must be pushed upward against gravity.

When the calf muscles contract—through walking, flexing, or movement—they compress the deep veins in the legs, propelling blood back toward the heart.

This is supported by one-way venous valves that prevent backflow.

This action is vital in venous return, especially when a person is upright. If the calf muscles remain inactive for extended periods (such as during long flights or sedentary lifestyles), blood can pool in the lower legs, increasing the risk of deep vein thrombosis (DVT) or venous insufficiency.

Medical experts and studies from institutions like the American Heart Association confirm the importance of regular movement and calf activation in promoting healthy circulation and preventing blood clots.
