Church Health Services, Inc., a 501c3 and a United Way agency, conducts primary, dental and mental health care clinics for low income children and adults who live in South Central Wisconsin. We also partner with local congregations to help them to develop congregational health ministry teams and manage Parish Nursing programs in participating congregations.

Did You Know We Have a Charitable Pharmacy?

We are proud to offer a Charitable Pharmacy, managed by Dennis Breidenbach, RPh, and Bev Beal-Loeck, Pharmacy Assistant. Both generously donate their time to help provide prescription medications, medical supplies, and equipment to those in need.

Because our pharmacy is staffed by volunteers, hours are limited, and we may not always have every medication available.
If you are in need of a prescription, please have your doctor fax it to 920-887-2322 or call 920-887-1766, option 201 or 1. We will let you know if we are able to help.

We often have diabetic supplies, medications, personal care items, and durable medical equipment.

While financial donations are welcome to support our work, they are never required.

If you or someone you know could benefit from these services, please reach out. We are here to help!

Use the Dental Clinic extension 201 to leave a message. If we have a medication you need, all your doctor has to do is fax us your prescription.

Like any dental clinic, our equipment and supplies need regular replacement. However, as a charitable dental clinic, we do not have the same income stream as traditional practices. To continue providing high-quality, compassionate dental care to individuals who rely on our services, we are asking for your support.

Welcome to our new Behavioral Health Staff!

We are excited to welcome three new counselors who will be supporting students and families in our school communities.

Nikki Ferriol joins Church Health Services as a Clinical Substance Abuse Counselor (CSAC), collaborating with middle and high schools on prevention and early intervention efforts. She brings over 30 years of youth advocacy experience.

Rachel Hau joins our team as a School-Based Mental Health Therapist serving the Waupun School District. A state-certified therapist working toward her LCSW, Rachel is passionate about helping students feel supported and build skills for long-term success.

Justyce Wandersee joins our team as a School-Based Mental Health Therapist serving the Beaver Dam Area School District. She is passionate about helping clients recognize their inner strengths, discover purpose, and feel empowered in their lives.

On Giving Tuesday, we were delighted to welcome a visitor who arrived with a generous donation of beautifully prepared gift bags for both adults and children. Laura shared that her wish this Christmas season was to “let her light shine for others,” and her kindness certainly did just that. 🌟 Help Us Keep Our Little Free Pantry Stocked! 🌟

At Church Health Services, our Little Free Pantry is open to the community, offering non-perishable food items at no cost to those who need a little extra help.

With an increase in community need recently, your donations make a big difference!

We gladly accept non-perishable food donations, feel free to drop them off directly at the Little Free Pantry, located on the Maple Ave side of our building.

For larger donations, please stop by our front desk and see Jamie, or give her a quick call at (920) 887-1766 ext. 214 to arrange a drop-off.

Every can, box, and bag of food helps brighten someone’s day thank you for helping us continue to care for our neighbors!
