Why waiting until goal weight is a mistake:

Opaque pricing in medical aesthetics isn't a business strategy. It's a patient rights issue.

When a practice requires a consultation to reveal pricing, or discloses the price for the first time when a patient is already in the treatment chair, that is a deliberate mechanism designed to bypass a patient's ability to make a free and informed decision.

Here's why it's an ethical failure:
It removes informed consent. Informed consent requires that a patient have access to all material information before agreeing to a procedure, including cost. A patient who doesn't know what something costs before agreeing to receive it has not provided truly informed consent. They've been managed into a decision.

It exploits the treatment room. There is significant psychological evidence on what happens to decision-making when a person is already physically committed to a situation. Lying in a chair, provider in the room, procedure about to begin, the social cost of saying no is high. Practices that reveal pricing at that moment know this.

It's predatory toward new patients. Patients who are new to aesthetics and unfamiliar with typical pricing are the most susceptible. The patients least equipped to push back are the ones most likely to be in that chair for the first time.

Transparent pricing is not a competitive differentiator. It is a baseline ethical standard that every medical practice should meet, and most in this industry don't.

Tipping.
Most medspas, particularly the discount and chain practices, operate on a tipping culture. Providers are paid partly or primarily through commission, which means they depend on gratuity to supplement their income. Patients who don't know this feel the social pressure at checkout. 15–20% on a $300 treatment is $45–60. On a $700 filler appointment it's $105–140. On a $1,500 Ultherapy treatment it's $225–300.
That is a significant cost that is invisible until you're already in the chair.

Neurotoxin duration varies meaningfully between patients, and not always for the reasons patients assume. The same dose administered to two patients of similar age and treatment area can last 3–4 months in one and 6–8 weeks in another. Dosing is one variable. Here's the clinical connection most practices don't explain:
UV radiation and collagen breakdown.
UVA and UVB radiation trigger the production of matrix metalloproteinases, enzymes that actively degrade collagen and elastin in the dermis. RF microneedling does not routinely destroy facial fat or collagen. This claim needs a clinical correction.

There's a partial truth being significantly misrepresented. Yes, RF energy at high depth and temperature settings used intentionally for body contouring can affect adipose tissue. That's the mechanism behind fat reduction devices. But that is not how facial RF microneedling for rejuvenation is performed.
Facial rejuvenation protocols target the dermis at 1–3.5mm depth. Controlled heating to 40–42Β°C stimulates fibroblast activity and collagen neogenesis, the opposite of destruction. Subcutaneous facial fat sits beneath the dermis. At appropriate settings it is not the target and not the outcome.
Fat atrophy has been documented as an adverse event, in cases of aggressive parameters, excessive passes, and undertrained operators. That is a training and oversight problem. It is not evidence that the modality is inherently harmful when used correctly. A surgeon can cause harm with a scalpel. We don't conclude that surgery destroys tissue.

The irony: the majority of practitioners amplifying this claim don't own an RF device and haven't been trained on one. Nuance matters in medicine, especially when it's scaring patients away from treatments with substantial peer-reviewed evidence behind them.

PM Aesthetics uses Symplast, an electronic health record (EHR) system built specifically for plastic surgeons and surgical practices. It is a HIPAA-compliant medical records platform, not a spa scheduling app. However, it's not cheap.

Here's why that distinction matters for you as a patient:
Your photos are medical records. Before and after photos at PM Aesthetics are stored in a HIPAA-compliant EHR, documented, trackable, and protected under federal medical privacy law. At a practice running Vagaro or similar spa software, your photos may be stored with the same level of protection as a yoga studio's class schedule.
Your chart is a real medical chart. Treatment history, product lots, dosing records, provider notes, informed consent documents, adverse event documentation, all stored in a system designed for medical practice, not retail booking. If something happens, we have a complete clinical record.

Your data is protected. HIPAA applies to medical records held in medical systems. It does not automatically apply to data held in general-purpose booking platforms. Knowing what system holds your health information, and what standards govern it, is your right as a patient.

Private equity has been quietly acquiring independent medspas for the past several years, and most patients don't know it's happened until they feel the difference.
Here's what actually changes when a practice gets acquired:

The providers leave. PE-backed consolidators optimize for margin. Experienced, higher-compensated providers get replaced with lower-cost staff. The injector who knew your face, your history, your response to product, gone. You start over with someone new who's working a schedule designed by a spreadsheet.
Recommendations become revenue-driven. When a practice operates under quarterly revenue targets set by an investment committee, clinical recommendations don't happen in a vacuum.
