Vital Records
Birth certificates are $10. Death certificates are $11. Immunizations
Most insurance, Medicaid, no insurance or underinsured is accepted here. 317 is for adults that don't have insurance, and is $15 per shot. Underinsured is $15 per shot. The VFC (Vaccines for Children) program we provide here as well, and that is also $15 per shot. If you can't afford to pay, we can and are willing to waive the fee. If you have any questions, please call us at 812-275-3234, extension 2613, Monday thru Friday, 8:30am-4:30pm. Our office is closed for lunch daily from 12 noon- 1:00pm.

Did you know? We offer FREE COVID-19 test kits in the nursing division lobby during normal business hours (M-F 8:30am-4:30pm, closed from 12pm-1pm for lunch). We urge parents to take steps to protect the health and safety of their infants following a surge this month of deaths caused by unsafe sleep environments.

Unsafe situations often involve co-sleeping with an adult (such as in a bed), blankets and pillows in the baby’s crib, or the baby being placed on his or her stomach to sleep.

Remember the ABCs of safe sleep:
1️⃣ Alone: Babies should be placed alone with caregiver nearby in the same room
2️⃣ Back: On his or her back
3️⃣ Crib: With only a tight-fitting sheet, no bumper pads, pillows, blankets or stuffed animals
