Brooke Waybright, Director of Quality & Compliance, recently sat down with us to share the ins and outs of what our Patient-Centered Medical Home (PCMH) recognition means for our patients and community.
Q: What is a Patient-Centered Medical Home?
Brooke: A Patient-Centered Medical Home is an organization where the patient is at the center of everything we do. Patient care is coordinated, team-based, and focused on the whole person, not just treating the illness.
Q: What does this recognition mean for our patients?
Brooke: This recognition means that our patients have better access to their care team, improved communication, and a more personalized healthcare experience. It shows that our organization meets national standards for quality of care, and our patients have a care team that knows them, listens to them, and works together to meet their individual needs.
Q: How does the PCMH model improve the care patients receive?
Brooke: This model improves care by strengthening communication between our patients and their healthcare team, enhances care coordination, and focuses on prevention and management of chronic diseases. We are following up on patient needs, reducing gaps in care, and providing support - all which lead to improved health outcomes.
