During her visit, she shared guidance on:
โ Healthy snack choices
โ Strategies for managing blood sugar
โ Everyday habits that support long-term wellness
This is what School-Based Health Center care looks like in action, meeting students where they are and equipping them with tools to confidently manage their health.
At CHWP, we believe prevention and education are just as important as treatment. Through strong partnerships with our local school districts, weโre helping build healthy futures for every child.

This yearโs theme came to life in simple, intentional ways:
๐จ Creative Corner Activity
Students and staff added drawings, positive notes, and thank-you messages to our health center doors โ turning them into vibrant reminders of connection and support.
๐ง Wellness Welcome Day
Students received calm kits filled with stress balls, fidgets, coloring pages, and calming resources to encourage healthy coping strategies all month long.
๐ Kindness & Connections Day
We delivered appreciation snack baskets to our school partners as a small thank-you for the big role they play in supporting student health.
These activities may seem small, but together they reflect something powerful: when schools and healthcare work hand in hand, students thrive.

CHWP's School-Based Health Center nurse, Tyler Shafer, LPN, recently spent time in Benjamin Logan Local School District classrooms teaching students about proper handwashing and staying clean to help prevent the spread of germs. ๐งผ๐ฆ
We love to bring education directly into the classroom, helping students build lifelong healthy habits and keeping our school communities stronger together. ๐

Yesterday, CHWP's President/CEP, Tara Bair, attended Live2Lead 2026 alongside the Champaign County Chamber of Commerce and other local leaders throughout Champaign County.
One of her biggest takeaways?

Through the teachings of John C. Maxwell and insights from local leaders Patrick Trenor and Geron Stokes, Tara was reminded that leadership starts with listening โ not just watching โ and continually growing.

Welcome to Community Health & Wellness Partners (CHWP), a patient centered medical home. As your community partner in healthcare, we provide economic-based, whole person care for long-term health and wellness to all the citizens of Logan and surrounding counties.
Community means Everyone. CHWP offers a full-range of healthcare services at three locations, Bellefontaine, West Liberty and Russells Point, and serves all persons--the insured, uninsured*, underinsured, and self-pay. (A reduced payment system, known as a Sliding Fee Scale may be available based on household income.) *Because we are able to offer care for a broader range of health insurance coverages, CHWP provides additional assistance to uninsured patients to enroll in Medicaid, CHIP, and other programs.
Community is at the Core. As a federally-qualified health center, CHWP has joined a network of over 40 health centers in Ohio and over 1,800 nationally, all committed to serving citizens in their respective communities. We work with Mary Rutan Hospital to reduce the burden on their emergency room through the implementation of patient care programs which ultimately improves patient health outcomes and reduces costs associated with the treatment of chronic illnesses.
Community has a Voice. Through a Board of Directors comprised of citizens of Logan County, there is A COMMUNITY VOICE in the operation of CHWP which allows the organization to be guided by consumers' opinions, thoughts, and ideas.
Community Investment. We are focused on providing the best healthcare possible and we recognize that in order to do so, we must also invest in our community through economic development and the growth of our organization.
