Yesterday, CHWP's President/CEP, Tara Bair, attended Live2Lead 2026 alongside the Champaign County Chamber of Commerce and other local leaders throughout Champaign County.
One of her biggest takeaways?
✨ Know your “why.” When you understand your purpose, you lead with clarity and intention.
Through the teachings of John C. Maxwell and insights from local leaders Patrick Trenor and Geron Stokes, Tara was reminded that leadership starts with listening — not just watching — and continually growing.

A 𝗻𝗲𝘄 𝗼𝗽𝘁𝗶𝗼𝗻 𝗳𝗼𝗿 𝗰𝗲𝗿𝘃𝗶𝗰𝗮𝗹 𝗰𝗮𝗻𝗰𝗲𝗿 𝘀𝗰𝗿𝗲𝗲𝗻𝗶𝗻𝗴 𝗶𝘀 𝗮𝘃𝗮𝗶𝗹𝗮𝗯𝗹𝗲 at CHWP.
Some patients may now be eligible to collect their own HPV screening sample during a clinic visit — no pelvic exam required.

Due to the extreme cold, the Mobile Health Unit visit originally scheduled for Tuesday, February 3, 2026 at the Eagles Annex in the Chippewa Neighborhood has been canceled. Second Harvest Food Truck distribution is also canceled.

Welcome to Community Health & Wellness Partners (CHWP), a patient centered medical home. As your community partner in healthcare, we provide economic-based, whole person care for long-term health and wellness to all the citizens of Logan and surrounding counties.
Community means Everyone. CHWP offers a full-range of healthcare services at three locations, Bellefontaine, West Liberty and Russells Point, and serves all persons--the insured, uninsured*, underinsured, and self-pay. (A reduced payment system, known as a Sliding Fee Scale may be available based on household income.) *Because we are able to offer care for a broader range of health insurance coverages, CHWP provides additional assistance to uninsured patients to enroll in Medicaid, CHIP, and other programs.
Community is at the Core. As a federally-qualified health center, CHWP has joined a network of over 40 health centers in Ohio and over 1,800 nationally, all committed to serving citizens in their respective communities. We work with Mary Rutan Hospital to reduce the burden on their emergency room through the implementation of patient care programs which ultimately improves patient health outcomes and reduces costs associated with the treatment of chronic illnesses.
Community has a Voice. Through a Board of Directors comprised of citizens of Logan County, there is A COMMUNITY VOICE in the operation of CHWP which allows the organization to be guided by consumers' opinions, thoughts, and ideas.
Community Investment. We are focused on providing the best healthcare possible and we recognize that in order to do so, we must also invest in our community through economic development and the growth of our organization.
