Welcome to Bend Transitional Care – the modern choice for skilled nursing and rehabilitation services in Deschutes County and communities throughout Central Oregon. Located at 2366 NW Lakeside, our brand new facility is located just yards from St. Charles Medical Center, which allows us to provide the continuity of care necessary to rehabilitate you or a loved one after an injury or illness. Physicians have easy access to visit patients under their care and greater communication between healthcare professionals at all levels of care.

Our mission is to enhance the life of every person we serve. We accomplish that goal by crafting a care plan specific to your needs and working diligently to return you to independence. Our newly built rehabilitation center was designed with the patient in mind. You will be surrounded with the comforts of home and the latest in nursing and rehabilitative technologies while our professionals work with you toward a speedy and comprehensive recovery.
