If you’re caring for a parent, save this checklist. Most families wish they’d had it sooner.
It usually starts small.
Then one day you look up and realize you’re managing all of this. Take a momemt to check the boxes and add up the score.
☐ Appointments scheduling, tracking, following up
☐ Medications changes, refills, interactions, side effects
☐ Transportation to appointments, procedures, therapy
☐ Meals and nutrition what they’re eating, whether it’s enough and if its nutritious
☐ Fall risks such as bathroom safety, mobility, home setup
☐ Memory or cognitive changes and what they mean
☐ Provider communication making sure everyone’s on the same page
☐ Your own life job, family, your own health
Now count how many you checked.
2–3: You’re already a care coordinator. You just don’t have the title.
4–5: You’re in the thick of it. This is a lot.
6+: You needed support yesterday. Please don’t wait.

2. Focus there first you don’t have to solve everything at once
3. Talk to someone who knows how to coordinate care, not just provide it
Most families don’t need more effort.
They need better coordination ad “help care”.
