As of July 1, 2022 the Benton location will be permanently closed. Dr. Carmen Jacoby and staff greatly appreciate all of our patients and have enjoyed the opportunity to treat you. Dr. Carmen will not be seeing patients in our West Frankfort. However, if you choose to see one of the doctors at that location they are more than happy to do so. To schedule an appointment at that office please call 618-937-3509. Our last day to see patients is Monday, June 20, 2022 (we will not be in the office on Friday, June 17, 2022). Again a great thanks to all of the patient we have treated over the years.
