Is your loved one paying privately to live in a facility?

Do you worry that they are not receiving the level of care they deserve?

Pleasant Hill Care Home has a private room available. PHCH is designed to provide individualized care. We provide 24 hour care in private rooms that your loved one may not be able to receive in a nursing home or assisted living facility. The care home is set up to be a resident’s home away from home and you decorate their room with their personal belongings so their room is more homelike and comforting. We also have a large dining area, TV area, and back patio area so that residents have a place to enjoy outside of their individual rooms.

We provide spacious room, home cooked meals, medication management, handicap accessible restrooms, laundry service, and meet all personal care needs. PHCH coordinates with home health or hospice If the services have been ordered by
your loved one’s primary care physician. We can provide services to residence that may only require limited supervision to someone requiring total care. We have been in operation for over 10 years providing excellent care. We are happy to share our list of references. Call for more information.
