IMMEDIATE ATTENTION REQUIRED


Dear Current & Former Patients of the IVF Program at the Reproductive Endocrinology & Fertility Division of the OB/GYN Department located at Walter Reed Military Medical Center (WRMMC) in Bethesda, Maryland.

A.R.T. Institute of Washington (A.R.T.) currently operates the IVF Laboratory pursuant to a contract between A.R.T. and WRMMC. After 22 years, the Contract will end on December 31, 2022, and a new operator will assume operations of the IVF Lab on January 1, 2023. The new operator will be Shady Grove Fertility (SGF)


IMPORTANT NOTICE REGARDING FROZEN SPECIMEN(S) IN STORAGE

If you have frozen specimen(s) in storage at the IVF laboratory at Walter Reed Military Medical Center (“WRMMC”), these are your options with respect to your frozen specimen(s):

1) Discard,
2) Transfer to SGF & continue to store at WRMMC,
3) Transfer to a long- term storage facility, or
4) Transfer to another fertility center.

We ask that you contact us no later than November 30, 2022. If we do not receive your selection by then, A.R.T. reserves the right to transfer your frozen specimen(s) to a long-term storage facility.


UNCLAIMED FROZEN SPECIMENS


PLEASE BE AWARE THAT YOUR FROZEN SPECIMEN(S) MAY BE DISCARDED WITHOUT FURTHER NOTICE IF IT/THEY IS/ARE CONSIDERED UNCLAIMED. WE CONSIDER FIVE (5) YEARS AFTER THE LAST CRYO-STORAGE EVENT A REASONABLE PERIOD OF TIME TO DEFINE EMBRYOS UNCLAIMED IF WE HAVE NOT HEARD FROM YOU FOR FIVE (5) YEARS OR YOU HAVE NOT PAID FOR CRYO-STORAGE FOR FIVE (5) YEARS.

PLEASE BE FURTHER AWARE THAT WE CONSIDER REASONABLE EFFORTS TO CONTACT YOU AS AT LEAST TWO (2) ATTEMPTS OVER THE YEARS TO CONTACT YOU EITHER IN WRITING, BY EMAIL OR BY TELEPHONE.

Hey IVF -
All baseline and monitoring appointments for tomorrow- Jan 4th will be as scheduled @ 0700.
OPM may call a 2hr delay but we will be open at 0700 in the GYN clinic.
