Grateful for all the kind words! ✨
My patient unfortunately came to me with a common story- he was left with a distorted nasal tip AND breathing difficulties after multiple prior rhinoplasty surgeries by another surgeon many years ago. His previous surgeon had used septal cartilage and ear cartilage grafts, but my patient’s nose still had poor structural support.
My patient wanted to improve both the function AND appearance of his nose. Specifically, he wanted to improve his:
❌ Breathing difficulty
❌ Distorted tip
❌ Asymmetry
To correct both his functional AND cosmetic issues, I performed an open revision rhinoplasty with MTF fresh frozen cartilage grafts and mastoid fascia grafts. His case was incredibly difficult due to all the distorted anatomy, scarring, and abnormally placed and stacked grafts from his prior surgeries!
He required
✅ EXTREMELY meticulous & delicate dissection to separate all the structural components of his nasal framework and remove his previously placed grafts
✅ Release of all the scaring to mobilize his native nasal cartilage framework and restore the anatomy to its natural position
✅ Creation of a columellar strut graft and bilateral spreader grafts for structural support and for correction of his breathing issues
✅ Coating of the newly created rigid cartilage framework with soft moldable mastoid fascia grafts to further refine the shape and ultimately provide a natural appearing result

My patient came to me because he wanted to improve both the function AND appearance of his nose. Specifically, he wanted to improve his:

To correct his issues, I performed an open septorhinoplasty with nasal tip deprojection & mastoid fascia grafts. This addressed both his functional & cosmetic issues.

✅ Straightening of his crooked & asymmetric nasal framework

✅ Deprojection & refinement of his nasal tip to create a refined, masculine shape while still providing a balanced, proportionate result
✅ Coating of the newly created rigid cartilage framework with soft mastoid fascia grafts to correct his bifid tip, further refine the shape, & provide a natural-appearing long-term result

Behind the scenes of an Open Primary Septorhinoplasty to address both function AND appearance! Here I discuss the different types of cartilage grafts used in his surgery.
My patient had an old injury to his nose which left him with a severely deviated septum & obviously crooked nose. He had chronic breathing difficulties since his injury, which he wanted to fix. He also wanted improved definition to his bulbous tip. To correct his problems, I performed an open septorhinoplasty.
He required:
✅ EXTREMELY careful dissection & removal of his previously injured & distorted septal cartilage in such a way that it could still be used to create the necessary cartilage grafts
