Potential Risks with Certain Uses of Radiofrequency (RF) Microneedling – FDA Safety Communication

The U.S. Food and Drug Administration (FDA) is making consumers, patients, and health care providers aware that serious complications have been reported with certain uses of radiofrequency (RF) microneedling devices.

These devices use an array of small electrodes, also known as microneedles, to deliver RF energy into and under the skin to produce local heating. With use of these devices for dermatologic or aesthetic procedures intended to improve the appearance of the skin (to treat wrinkles or to achieve skin effects, sometimes referred to as resurfacing, “tightening” or “rejuvenation”), the FDA is aware of reports of serious complications (adverse events) including burns, scarring, fat loss, disfigurement, and nerve damage, and the need for surgical repair or medical intervention to treat injuries.

While the FDA’s evaluation is ongoing, we are asking patients, caregivers, and health care providers to report any complications related to the use of these devices for dermatologic or aesthetic skin procedures to the FDA. Prompt reporting of adverse events can help the FDA identify and better understand the risks associated with medical devices.

Comment from Dr Fernandes:
“The risk of “overtreatment” and excoriation of the epidermis becomes a real danger when trying to do skin needling at 2.00 mm or more with these vertical mechanical needling devices. The needles have to “glide” over the skin while they pe*****te, and it becomes difficult to get vertically oriented needles to pe*****te 2 mm and to be fully withdrawn as one glides over the skin. If the needle has not been fully withdrawn as one moves, then the needle could damage the epidermis that is only 0.2 mm thick (ie, just 10% of the needle length). The mechanism of skin pe*******on to needle the skin safely with a mechanical device needs to be understood. Skin resists being pe*****ted and there is a friction on the needle surface both going in and coming out of the skin. This can be seen with slow motion, extreme close-up videos. As the needle impinges on the surface, the skin gets pushed down and then the needle pe*****tes to its “dialed” length and then starts to be withdrawn. Friction starts to pull the skin surface upward and ultimately the needle exits the skin.”

After needling the face and neck, I thread very high quality Seirin™ Japanese needles underneath wrinkles in order to soften lines. The subcutaneous transversal micro-trauma in the dermis will induce collagen production and therefore plump up any depression. This technique can be used on cross feet, glabellar lines, nasolabial folds, marionette lines and wrinkles on the forehead. It complements needling beautifully, addressing additional areas of concern for a comprehensive rejuvenation. My signature treatment concludes with a 20 minute medical red light therapy session.
