👨🏻‍⚕️ When performing circumferential torso surgery, it is crucial to include the mid and upper back.

Skipping this area can lead to unbalanced results over time, becoming disproportionate, and the lower back may remain retracted while the upper back “rolls over,” creating an unnatural appearance.

One lesser-known symptom of Lipedema is pain or tenderness when putting on a blood pressure cuff.

Many of our patients describe discomfort or even sharp pain from simple pressure, whether it’s a cuff tightening around the arm, a pet jumping onto their lap, or even light touch in affected areas.

This sensitivity is due to the inflammation and fibrosis that occur within Lipedema tissue, making it painful to the touch. 🙋‍♀️ My lovely Stage 3 patient from Texas is back for her next stage of treatment.

Previously, she underwent surgery on her calves, ankles, thighs, and arms. Today, we plan to treat her circumferential torso. She’s already seeing incredible improvements, and has reported that her pain is completely gone.

She plans to address remaining skin laxity and vascular issues in future procedures, but her progress and transformation is already remarkable.

My patient has been on Trizepatide for 3 years, and we’ve talked a lot about its role in Lipedema.

While many patients may lose significant weight, and she herself has lost 140 pounds, those with Lipedema often notice less fat loss from the calves, ankles, arms, and affected areas, and more from visceral fat.

An interesting benefit of GLP-1 agonists is their effect on improving inflammation, which can be helpful even before surgery. However many patients can reach a point of being “maxed out,” ready to explore surgical options if needed.

👀 Sneak Peek into the Operating Room

This patient was experiencing swelling in other areas of her body caused by congestion in the torso.

During treatment, I discovered significant fibrosis in the torso that was blocking lymphatic flow and contributing to swelling elsewhere.

Not every patient will need torso treatment, for example Type 3 or Type 5 lipedema where the torso and pelvic girdle aren’t involved, but for this patient, addressing the torso was crucial for overall improvement.

✍️ Markings to the Arms

My patient came all the way from Australia for a comprehensive, 2½ stage surgical plan. Yesterday we treated her calves, ankles, and pelvic girdle, and today she underwent a partial arm surgery. She’ll return to town at a later date to complete her thigh procedure.

Performing partial surgeries in smaller or disparate areas a day or two apart allows it to be done safely, stay within safe limits of anesthesia and reduces number of trips to complete the surgical treatment plan.

🙋🏻‍♀️ Meet Gloria, an athlete, former figure skater who taught fitness and nutrition for over two decades.

Despite her dedication, her legs never seemed to respond, and catch up to the rest of her body. Even lacing up her skates or wearing boots became a struggle.

🦵 My patient is halfway through her circumferential thigh liposuction surgery and we’ve already removed 5 liters of fat.

After her last surgery, her calves and ankles look amazing, with reduced cuffing and improved definition. Now we’re tackling the knees, and releasing a lot of stubborn fibrosis.

Many surgeons hesitate to go near the knee cap, but it’s crucial because fibrosis often builds up at the knees and elbows. Already, her knees are starting to take beautiful shape, and we can’t wait to see the final transformation!

When comparing areas of fat in Lipedema, you can actually see a difference in coloring and texture.

⚪ Fat that appears more white is typically more fibrotic. It feels dense and firm, almost like cement.

🟡 Fat that looks more yellow, like chicken fat, is less fibrotic and has a softer consistency.

These differences matter, because the degree of fibrosis can impact symptoms, pain, and the surgical approach.
