Here is a case with a screw retained implant that is full gold. The alloy with this was Jensen Dental's Castell. It has 20%AU and so is considered Nobel rather than High Nobel. The trade off is that given the siz e of the restoration it is far more affordable than the High Nobel version. Both options are available upon request. Case success over a span of three decades is the primary justification for why we wax and cast everything in house. With the cost of alloys at an all time high, why skimp on the art and attention to detail?

This Bridge is an example of why PFMs will never completely die. When a patient breaks all other materials, and yes that includes even the best zirconia, Metal is the true last stop. The patient here wanted to maintain aesthetics yet needed the strength and stability of metal. The all-metal crown was opposing an all-metal crown and to maintain an even wear pattern we matched the occlusal material.
Though the need for PFMs will never entirely go away, the labs capable of fulfilling that need are dwindling. The techniques involved require a long and steep learning curve. Even waxing and casting framework alone takes time and no small quantity of talent to achieve Proficiency and Quality. The stacking of porcelain by hand requires time and talent in spades to acheive Proficiency and Quality. The training cost can be heavy on a lab and technician, and talent can wane if there aren’t enough cases coming in. At this point, most labs, if they have a technician who can do PFMs, will not be training another technician in the art.
However, we keep seeing a trend with PFM cases. Doctors who have come to us after several cases of broken long span bridges. They may have experienced a PFM bridge that was clearly made by a technician who is either out of practice or hasn't practiced enough. They are prepared to tolerate a low quality of aesthetic.

This case is a couple years old now but we still love to bring it back out now and again. Some of the greatest endorsements we get are from those who truly know aesthetics and are in the industry, choosing us to do their smile, or their family members smiles. Doctors who we work with constantly and even those who haven't worked with us in years will send us cases of their family. If you have the word family in your practice name, choosing us for all your aesthetic needs is a way to show you really do treat your patients like family.

Been a While since I featured our other top notch Lead ceramist and great artist Phill. This here is a sticker I made that I think capture a bit of our artists personality. Don't let the snakiness fool you, Phill is goal oriented, he wants the best possible outcome for every case he works with. To that end he is always available and motivated to discuss your cases should you need pick his brain. Phill has over 25 years as a technician and much of that operating his own lab, for years he was one of Billings best kept secrets, servicing a few old time dentists who demanded only the best.
