STIC Habilitation Services Community Habilitation services provide supports to both children and adults with developmental disab

The Habilitation Programs at STIC are person-centered, community and home based program. Habilitation staff are trained to work as a team with individuals, Service Coordinators, parent/advocates and family members. Individuals learn new skills while planning life goals which allow the individual to become as independent as possible. One-on-one services are provided to individuals with an emphasis on the likes, wants, needs and dreams of each individual in the program. Examples of activities, goals and skills commonly worked on in the Habilitation Programs include but are not limited to:

Social Skills
Community Integration
Cooking/Food Shopping Skills
Money Management Skills
Activities of Daily Living Skills
Basic Hygiene Skills
Interest Exploration
Public Transportation

🤔❓Who can you hire in CDPAP ?
✅Anyone over the age of 18 (except a spouse of the consumer or a parent if the consumer is under the age of 21).

❓What can a PA do for you in the CDPAP program?
✅Housekeeping
✅Bathing
✅Cooking
✅Shopping
✅Assistance with Mobility/transferring
✅Medication distribution
✅And so much more!

💡The tasks a PA can do are based on a plan of care which for each consumer will be different as it is based off of their needs.

🔦This week, we’re shining a light on STIC’s Consumer Directed Personal Assistance Program (CDPAP)!

🫂🤲Do you need home care? Do you want to direct your own care? There is a program for you!

🤔❓What is CDPAP? CDPAP is a Medicaid funded program that allows people who require personal care assistance to hire, train, supervise, schedule, and terminate their own Personal Assistants (including most family members, a neighbor or a friend). CDPAP is an alternative to traditional home care as it allows you to direct your own care.

✦This program works with people with disabilities of any age who have Medicaid and are approved by the New York Independent Assessor Program (NYIAP) for CDPAP services.

✦STIC is a CDPAP Facilitator partnered with Public Partnerships LLC (PPL), the Statewide Fiscal Intermediary (SWFI) for CDPAP.

Nonperishable food items, hygiene products, clothing, and reusable bags can be dropped off at 135 East Frederick St, upstairs in the reception area during our business hours of Monday through Thursday, 8:30am to 4:30pm, and Friday, 8:30am to 3:00pm.

The items we are most in need of are:

Our Story

The Community Habilitation Program at STIC is person-centered, community and home based program. Direct Support Professionals (DSP) are trained to work as a team with individuals, Service Coordinators, parent/advocates and family members. Individuals learn new skills while planning life goals which allow the individual to become as independent as possible. One-on-one services are provided to individuals with an emphasis on the likes, wants, needs and dreams of each individual in the program. Examples of activities, goals and skills commonly worked on in the Community Habilitation Program include, but are not limited to:

• Community Inclusion • Money Management • Situational Safety Skills • Cleaning • Socialization • Self Advocacy • Personal Hygiene • Money Skills • Cooking/Food Shopping Skills • Use of Public Transportation • Time Management • Personal Hygiene • Diet and Exercise • Volunteering

Eligibility/Funding

To participate in this service an individual must have a developmental disability, be eligible for Medicaid, and be enrolled in the Home and Community Based Services Waiver (HCBS). STIC Community Habilitation services are funded by the Office For People with Developmental Disabilities (OPWDD) through the HCBS waiver. Prior approval from the Broome Developmental Disabilities Services Office is needed to receive Community Habilitation.
