What you don’t see is the planning behind it.

The paradigm around the timing of facelifts is shifting. We’re seeing more patients at a younger age choosing to address signs of aging earlier, rather than waiting until their 50s or 60s. Here is the secret to achieving refined facial rejuvenation - better balance.

At 6 months post-op, you can see this young woman’s transformation wasn’t about changing her identity. It was about refining proportions and improving how each feature relates to the next.

Kristin and I had the opportunity to build anywhere, including our hometown of New Orleans, but we chose Birmingham with intention. There is something special here. A quiet excellence, a strong sense of community, and a level of sophistication that continues to inspire us.

5 Months Post-Op: Preservation Extended Deep Plane Facelift/Neck Lift, Upper Blepharoplasty, Fat Grafting, Targeted CO2 Laser

Meet Catherine, perhaps one of the kindest sweetest people I know. I’ve had the pleasure of knowing Catherine since I was a resident at Tulane in New Orleans over a decade ago.

When Catherine reached out to me with an interest in facial rejuvenation, I was absolutely thrilled to rekindle the relationship but also help her through her facelift journey. Although Catherine works in the medical field and has extensive knowledge and experience seeing facial rejuvenation procedures, she told me it had taken her several years to build up the courage and take the leap of faith that is facial plastic surgery.

For Catherine, we performed a number of procedures to restore her youthfulness in a subtle, yet natural way. We performed our preservation extended deep plane facelift and neck lift to enhance her jawline, a precision upper lid blepharoplasty to debulk excess skin and accentuate her eyes, and targeted autologous fat grafting to add subtle volumization and contours. Lastly, we relied on our CO2 fractional ablative laser to bring out the youthful glow to her under eyes.

Why do we use surgical drains and netting sutures?

These are temporary but important parts of the healing process.

Surgical drains are placed to prevent fluid from collecting beneath the skin in the early post-operative period. By removing this fluid, we help the tissues heal more smoothly and reduce the risk of complications.

Netting sutures are used to support the skin. They help the tissues settle evenly and protect the work that was performed beneath the surface.

While these can look unfamiliar or even concerning at first, they are placed intentionally to optimize healing and improve overall outcomes.

Thoughtful surgery extends beyond the operating room, it includes every step of the recovery process.

This beautiful patient in her 40s sought to refresh her appearance while fully preserving her natural features. For select patients in the early stages of facial aging, a deep-plane facelift and neck lift can offer the most elegant and long-lasting approach to rejuvenation.

Because tissue quality remains strong at this stage, healing is often more efficient, and results integrate seamlessly. These patients are able to enjoy their outcomes during their prime years, and by effectively “resetting” the clock, they may continue to look naturally younger for decades to come.

Why does the neck often age faster than the face?

First, the skin of the neck is naturally thinner than the skin of the face, making it more susceptible to laxity and visible aging over time.

Second, the neck receives the same amount of sun exposure as the face, yet it is often overlooked when applying SPF. This leads to cumulative sun damage that accelerates skin aging.

Third, the platysma muscle bands in the neck are not supported by underlying bony structures, which allows them to become more prominent as tissues weaken and descend.

Together, these factors contribute to earlier and more noticeable aging in the neck.

Skin quality can be improved with proper skincare and treatments such as laser resurfacing or chemical peel. Structural aging, what we see as sagging, is best addressed by repositioning deeper tissues through procedures like a deep-plane facelift or neck lift. Volume loss, which leads to hollowing and deflation, can be restored with techniques such as fat grafting.

True facial rejuvenation is not one-dimensional. It is the combination of restoring structure, replenishing volume, and improving skin quality that creates balanced, natural results.
