2 Months Post-Op: Rhinoplasty & Targeted CO2 Laser

This patient's goal was to correct a dorsal hump and create a smoother, more refined nasal bridge. Through careful structural rhinoplasty, we were able to soften the profile while maintaining balance with the rest of the facial features.

At two months, healing is still in progress, and some residual swelling remains, particularly along the bridge and tip. Over the coming months, the nose will continue to refine as the tissues settle. I am grateful to this patient for allowing us to share her healing journey.

1 Year Post-Op: Rhinoplasty

When a boxer decides to hang up the gloves and fix his nose that’s endured years of hits, it’s both a challenge and an honor to step in as the surgeon.

This case was truly unique , in that this nose sustained years of repetitive trauma including an attempt at repair by another surgeon. What we found was significant scar tissue, asymmetry, distorted tissue planes, poor structural stability, a collapsed airway, and the need for significant reconstruction. Using rib cartilage, we rebuilt his nose, restoring a functional airway and creating a strong, masculine profile that’s natural and refined.

These complex cases demand full focus and precision, 2 hours and 10 minutes of surgical adrenaline, like my own version of a boxing match.

Rhinoplasty isn’t just about reshaping; it’s about restoring function, structure, and confidence. It’s a privilege to work on noses that tell such a story.

Every rhinoplasty starts here.

Careful evaluation. Surgical markings. A clear plan. This patient had a history of nasal trauma when she was younger. Over time it left her with a dorsal hump, a widened bridge, and noticeable crookedness from the front view. Her tip cartilages were thick and curved, which created a bulbous shape and softened definition, especially with a thicker skin envelope.

Functionally, she had lived for years thinking her breathing issues were “just sinuses.” In reality, she had a severely deviated septum causing significant obstruction.

Our goal was simple in concept but complex in ex*****on. Create a nose that feels refined, feminine, and balanced with her small frame… while also restoring proper nasal airflow. In surgery we corrected the septal deviation, narrowed and straightened the bridge, refined the tip structure, and repositioned the nasal framework back toward the facial midline. Structural grafting was used to ensure the result remains stable and natural for decades.

The images and intraoperative views show the early refinement. The real beauty of rhinoplasty isn’t making a new nose. It’s creating the one that should have been there all along.

“Scarless” facial plastic surgery is a common phrase, but in reality, surgery requires access.

Whenever we perform a procedure, small incisions are necessary to reach the structures we are operating on. Those incisions inevitably create scars. The goal of modern facial plastic surgery is not to promise that scars will not exist, but to place them thoughtfully and allow them to heal in a way that makes them difficult to notice.

Fat Grafting Explained: Placement Matters. With aging, volume loss occurs in predictable areas including the temples, tear troughs, cheeks, midface, nasolabial folds, and marionette lines. In select patients, subtle enhancement of the chin or lips may also improve overall balance. Strategic placement restores soft tissue support and contour without creating unnecessary fullness.

When paired with a facelift, fat grafting plays a critical complementary role. A facelift repositions descended tissue, while fat grafting restores the volume that has diminished over time.

1 Year Post-Op Periorbital Rejuvenation| The Same You, but Better.

This kind gentleman in his early 60s came to us seeking a refreshed appearance, not a transformation, but a restoration. He’s healthy, active, and takes pride in how he presents himself, yet his eyes and upper face were sending a message that didn’t reflect how he felt inside. Heavy brows, upper eyelid fullness, and excess upper lid medial fat pad contributed to a tired, stern, and even distant appearance.

There was also a mild functional concern: the heaviness over his outer upper lids was beginning to infringe on his superolateral visual fields.

With precision and intention, we performed a comprehensive periorbital rejuvenation, including:

- Endoscopic Brow Reset via a minimally invasive, endoscopic approach via hidden incisions within his temple hairline, lifting the brow subtly to reduce forehead strain, restore brow shape, and create facial balance.

Most people look at before-and-after photos and focus on what changed. I look at what didn’t.

49-year-old woman.
6 months after brow lift, upper lid blepharoplasty, fractional CO2 laser resurfacing, and a subtle liquid rhinoplasty.

No makeup in either photo. Standardized positioning as closely as possible. The after is slightly overexposed, but the details still speak.

Notice her brow position. It isn’t dramatically higher. That’s intentional.

She never had highly arched brows, and she didn’t want them. Modern brow rejuvenation isn’t about lifting everything upward. It’s about restoring support to the midface, relieving compensatory forehead tension, and softening upper lid redundancy, especially medially and laterally.

She looks more relaxed. More open. More approachable. That’s the goal.

The upper lids show the nuance. Excess skin was removed. Herniated fat was repositioned for smoother contour. Tarsal plate show and symmetry improved. Brow fat pad support restored. These are small decisions that create a natural eye.

Her skin quality is another quiet shift. Fractionally ablative CO2 remains the gold standard for tone, texture, pigment, pore size, and fine lines. I especially love what it does for lower lid crepiness.

A note on scars. Her hair is pulled back. No makeup. You can’t see the temporal brow access incision. That’s by design. There is a faint pink line near the tail of the upper lid. At 6 months, that’s normal. Some scars fade quickly. Others take closer to a year. Sun protection, silicone gel, and time matter. If needed, vascular lasers can help.

Finally, the most subtle change. A firmer hyaluronic acid filler was used to refine the nasal profile. Slightly more lift and definition to the tip. Nothing dramatic. Just better harmony.

This is what modern facial rejuvenation should look like.

3 Months Post-Op: Rhinoplasty

Healing is well underway, though refinement continues. At this stage, residual swelling, particularly along the tip and supratip, is still present and will gradually improve over the coming months.

This young patient of Middle Eastern background traveled from Arkansas seeking a softer, more feminine profile while preserving the character and identity of her nose. Our approach focused on balance and subtle structural refinement: smoothing a dorsal hump, refining supratip fullness, elevating a ptotic tip, correcting nostril retraction, and sculpting a bulbous, under-defined tip.

From a technical perspective, this case required a precise structural strategy. Using a small incision along the columella, we performed an open rhinoplasty to allow meticulous cartilage reshaping. Her own cartilage created a stable, long-lasting foundation, while a delicate layer of temporalis fascia added a soft, seamless cushion over the dorsum, ideal for patients with thick skin seeking a smooth, graceful contour.

Functionally, her breathing was also improved by addressing the internal valves with spreader grafts and optimizing lower cartilage positioning to enhance airflow.

While still early in the healing process, the foundational changes are already evident. Continued refinement over the next several months will further reveal the final result.
