The Centers for Disease Control and Prevention (CDC) estimates that 1 in 6 Americans gets sick by consuming contaminated foods or beverages each year. These are a common public health problem and often go unreported.

If you think you became sick from something you ate, please let us know. Your information is reviewed by the health department and may help in outbreak investigations. You can report your illness in a few ways:

1. Submit a report online using I Got Sick. This is a web-based reporting system that reviews your symptoms, places you visited, and what you ate. https://pubredcap.health.utah.gov/surveys/?s=AMR9NWM7YP8FFDF3
2. Submit your complaint by phone, by calling (435) 587-3838. A staff member will assist you in completing the I Got Sick form.

For general questions regarding foodborne illnesses, please call (435) 587-3838.

Please fill in all fields as completely as possible. The information you provide will help us determine if your illness is food related and what steps need to be taken by the appropriate Local Health Department.

Role Description

This is a part-time, on-site role for an Office Associate, located in Blanding, UT. The Office Associate will perform day-to-day tasks including providing assistance in vital records, assisting with customer inquiries, and handling clerical duties. This role requires exceptional organizational and communication skills to ensure the smooth functioning of the public health office.

Qualifications

MUST adhere to strict confidentiality requirements
Strong communication and customer service skills, including the ability to engage with diverse groups effectively
Experience in an office atmosphere is preferred
Confident in in asking questions to gain clarity for information needed.
Proficiency in relevant office software and technologies
Ability to work independently and collaboratively in a professional setting
High school diploma or equivalent required; additional certifications or education in administration or a related field is a plus.
