Thankful this morning for so many things! Family and friends of course…but there are so many other things to be grateful for! My family away from home—my dental team—is such an amazing group of people that I would struggle without! Thank you for your continued help daily. Serving our patients is of the utmost importance always and I know you all strive each day to be your best! I am thankful for the wonderful technology that has been created to assist us in helping our patients save their teeth! Microscopes, 3D radiography, apex locators and especially the Gentlewave by Sonendo. It is easy, as an endodontist, to think about the challenges we face each day but these systems have made our lives so much more efficient. Thank you! This case is a 4 month follow up on tooth #2. I am super thankful the patient returned to allow me to assess the healing. He stated there was absolutely NO pain after the root canal! Healing is not quite complete, but on its way. The tooth was feeling back to normal right after the procedure. Thank you Gentlewave! Everyone, remember to be grateful for all of the little things! Challenging case to finish the week! Tooth 19. Nearly completely calcified Mesial Buccal canal. Necrosis with AAP. Intra-osseous anesthesia required. Wow! Tough tooth! Patient was highly motivated to keep the tooth since the crown was just placed about 6 months prior. NOTICE THE SEALER COMING UP INTO THE LOWER PART OF THE MESIAL BUCCAL CANAL SINCE IT JOINED WITH THE MESIAL LINGUAL CANAL…also, note the sealer at the distal canal apex…no way to know how many portals of exit a root may have. My hunch is this is gonna do well!!!! THANKS AGAIN !!!

at it again! From what I could tell by looking at the pre-operative CBCT, this lower pre-molar only had one canal. The post-op periapical image revealed a tiny branch at the apex. Q: How many teeth are like this one??? A: We have NO IDEA! Therefore, use the Gentlewave on all cases when possible!

Two lessons from this case. 1) The patient stated his tooth had been tender to biting pressure for a few years. Not ever too serious. His dentist had noticed a slight fracture over the occlusal (chewing) surface for quite some time and had recommended a crown a couple years ago. Fast forward a few years to now. The patient stated he bit down on a little seed from a bagel and felt lots of pain. “The straw that broke the camels back”! Now it’s too late and the tooth must be extracted. 2) Cone beam computed technology allows us to see things we cannot see on normal radiographs. But even so, really small hairline cracks can’t be seen in the early stages. So if your dentist ‘sees’ a small crack in your tooth and you are having some tenderness eating certain things, you may want to have a CBCT scan to make sure there is no visible sign of vertical root fracture, then consider having a crown placed to keep the crack from getting worse. It’s hard to tell sometimes, but a crown in the early stages of a crack can help protect the tooth and preserve it for years to come. Go see your dentist!!!

GENTLEWAVE to the rescue again! No worries! The Apex locator gave decent readings on mesial and distal roots but the files were challenged by the length, narrowness and curvature. After running the and drying the canals, the Hi-flow was able to find and fill the apical openings. Wanted to highlight the decay on the distal under the crown. Recommended a new crown when the tooth is feeling better. I suspect a good long term prognosis! Thanks !!!

This young man (13 y.o.) returned today for a follow up exam on tooth #8. He had fractured the crown from a fall back in 2017. The pulp (nerve) was exposed due to the fracture. At 7 years old, he was very brave and let us open up the pulp chamber a bit which allowed us to place calcium hydroxide in the root then glass ionomer to close the access. After two years, the pulp was already forming a calcium bridge and the pulp tissue remained vital superior to the bridge. So now, four years later, the tooth seems to have fully developed and the patient is asymptomatic. This shows how, with minimal treatment for crown fracture, immature teeth can survive and grow into a normal healthy tooth! Who knows what the future will hold, but so far, so good!!!

With a specialty in endodontics, Dr. Spires attained a combination of skills such as: management of patients with orofacial pain and anxiety, vital pulp management, prevention, diagnosis, and treatment of diseases and injuries of the dental pulp and related tissues.

Dr. Spires earned his Board-certification through the American Board of Endodontists. According to the American Association of Endodontics, only 24% of endodontists are Board-certified putting Dr. Spires in a small percentage of qualified professionals.
