When a hospital stay is over, recovery may still be underway.

Calhoun Liberty Hospital’s Swing Bed / Transitional Rehab program provides short-term rehab and skilled nursing care to help patients regain strength before returning home - all close to the comfort of family and community.

As patients meet their goals and discharge from the hospital, availability changes. We currently have limited openings.

If your loved one may benefit from additional time to heal and rebuild strength, call our Case Management team at 850-674-5411, Ext. 250 to learn more.
