Columbus is pleased to announce our partnership with PocketRN and our participation in the Medicare Guiding an Improved Dementia Experience (GUIDE) model as a GUIDE Partner. Through this initiative, we are working to expand access to dementia care for individuals living with Alzheimer’s and dementia, as well as their caregivers. As part of CMS' eight-year pilot program, Columbus is contributing to efforts aimed at improving support and resources for those with a diagnosis of Alzheimer’s or dementia.

This partnership highlights our ongoing commitment to pursuing collaborations and innovative programs that advance care for the dementia community. We remain dedicated to empowering families with meaningful resources and compassionate support. 💜
