Life Care Concierge of South Florida Our mission is to bring you and your families heartfelt experiences to age in place. She is an expert in creating heartfelt experiences for aging-in-place.

Jill Poser-Kammet is a national leader in Care Management, Life Care Planning and Private Duty Home Care. The combination of her education with over 15-years of professional experience in the senior space and her own personal journey caring for her parents has led her to successfully establish Private Duty Home Care Concierge. She passionately listens and has created solutions for hundreds of families going through life-changing physical, emotional and cognitive decline of their loved ones. Jill strongly believes in the power of the Mind - Body connection in creating positive outcomes toward our long-term health and wellbeing. She is Care Manager Certified (CMC) through the National Academy of Certified Care Managers (NACCM); the preeminent nonprofit organization providing certification for Care Managers since 1995 and a Certified Geriatric Care Manager (CGCM) through the International Commission of Health Care Certification (ICHCC), an internationally recognized certification in care management. Both the CMC and CGCM credentials require strict adherence to continuing education, Code of Ethics and Standards of Practice. The ethical principles at the core of the Code of Ethics are the foundation for the Standards of Practice and sets a national standard for the very practice of care management.

Around that time, the National Association of Home Builders introduced a certification called Certified Aging-in-Place Specialist (CAPS), focused on serving older adults and individuals with special needs who wished to remain in their homes safely. I chose to pursue the certification and immediately fell in love with the work.
