We are looking for a Nurse Practitioner to join our growing team!
This role is both home based, and facility based geriatric primary care, with emphasis on advance care planning, palliative management of symptoms, and aging in place. Position also includes integration with the hospice team to maintain continuity and performing visits for hospice patients to help meet face to face requirements, and symptom management.

We are currently seeking an experienced and compassionate Mobile Primary Care Nurse Practitioner to join our growing team.
Position Overview:
As a Mobile Primary Care Nurse Practitioner, you will be responsible for providing in-home medical care to patients across ADA & Canyon Counties. You will work autonomously, collaborating with a multidisciplinary team to assess, diagnose, treat, and manage a wide range of healthcare needs. Your role will focus on providing personalized, patient-centered care to improve health outcomes for our diverse patient population.
Key Responsibilities:
• Perform comprehensive health assessments, including history taking, physical exams, and diagnostic tests.
• Develop and implement individualized care plans for patients with chronic conditions, acute illnesses, and preventive care needs.
• Prescribe and manage medications, as well as perform medical procedures within scope of practice.
• Educate patients and their families on managing health conditions and making healthy lifestyle choices.
• Collaborate with physicians, specialists, and other healthcare providers to ensure continuity of care.
• Document all patient interactions, diagnoses, treatment plans, and follow-ups in an electronic medical record (EMR) system.
• Provide in-home visits and telehealth consultations as needed.
• Maintain patient confidentiality and adhere to healthcare regulations and policies.
Qualifications:
• Current Nurse Practitioner (NP) licensure in ID.
• Board certification in Family Practice (FNP), Adult-Gerontology (AGNP), preferred. Palliative care experience and certification would be considered an excellent candidate.
• Minimum of 5. years of clinical experience, preferably in primary care, geriatrics, or home health.
• Strong clinical knowledge and the ability to work independently.
• Excellent communication, interpersonal, and organizational skills.
• Valid driver’s license and reliable transportation to travel to patient homes.
• Ability to adapt to a flexible and dynamic work environment.
• BLS/ACLS certification preferred.
