The Purpose of a Service Coordinator:
The following is a list of the duties and responsibilities of the Service Coordinator:
1. Service Coordination involves coordinating all services and supports across agency lines to ensure consistency of service delivery and to prevent duplication of over utilization of services.
2. The Service Coordinator will demonstrate impartiality.
3. The Service Coordinator shall serve as the liaison and advocate for the participant.
4. The Service Coordinator shall recognize the participant as primary decision makers unless otherwise exempted by the court.
5. The Service Coordinator will respect and protect confidentiality and will release information only with an appropriate written release.
6. The Service Coordinator will provide the participant or guardian with protection and advocacy, and legal assistance when appropriate.
7. The Service Coordinator is responsible to develop the Individual Support Plan. The Individual Support Plan will be developed using person centered planning methods and principles which acknowledge recipients and families as full partners in the planning and monitoring of services; support the recipient to define his/her personal future and realize self-determined individual goals; foster participation in community life, development of community skills, and relationships with other people in the community; and focus on the unique strengths and needs of each recipient and the utilization of informal, natural supports from the community.
