✔️ Collaborate with CHC staff and payers to drive better health outcomes and smarter spending

✔️ Use data to guide care management, cost control, and performance improvement strategies

If you have experience with value-based care, contract management, or quality improvement—especially in a community health or primary care setting—we’d love to hear from you.

She was selected for this award, because every day she demonstrates a strong commitment to improving access to care, removing barriers, and supporting individuals with compassion, consistency, and care. Everyone who has met Nurse Susan knows that she will do everything in her power to ensure that the patients she takes care of have access to the care and resources that they need. Every day she comes to work with so much determination to help all the people in this wonderful community, and that is truly a special quality.
