The Home Health Aide is responsible for carrying out assigned duties which promote patient comfort and support. As the Home Health Aide, you will provide personal care/hygiene to the home health patient and assist patients with activities of daily living.

Principal Accountabilities
Assist the patient with transfers, exercise and/or ambulation
Turn and position patients who are bed bound
Assist with Range of Motion appropriate for the patient as directed by the RN or Therapist
Assist patients with meals and nutritional support
Change bed linens and straighten patients’ environment
Observe and report changes in patient conditions to the case manager
Participate in interdisciplinary team meetings as appropriate to coordinate patient care, exchange information, and receive staff support and in-service education

Qualifications
Effective communication and interpersonal skills
Ability to work a flexible schedule, to include some evenings and weekends
Ability to travel to patient homes

As a nurse you will be servicing patients per day in the Chicago area. You will be expected to provide quality nursing care according to state laws and plan of care for each patient. You should have empathy toward patients, be responsible and responsive to patients' and employer needs.

Qualifications Of The Registered Nurse (RN)

Must have good standing license (RN) in the state in which the clinician will practice

Must have at least 12 months of hands on nursing experience

Current CPR certification

Willing to travel up to 100% of the time for business purposes is required

Responsibilities Of The Registered Nurse (RN)

One to one patient care in a home environment

Ex*****on of physicians prescribed plan of care and compliant documentation of care provided in system of record

Administration of prescribed medication, treatments and therapies

Patient assessments, coordination of care
Health, promotion, teaching, and training of family members

Initiates, develops and implements nursing plan of treatments; evaluating patient progress towards goals.
Organizes and participates in the provision of direct patient care, performs treatments, administers medications, and educates patients and families/caregivers.
Modifies plan of treatment in response to changing patient status or physician orders to achieve established or revised patient care goals.

Assesses patient needs and physical status at each skilled visit through health data access and patient interview. Re-evaluates patient needs through physical reassessment, response to therapy, and supplemental physician orders. Obtains and clarifies physician orders for plan of treatment revisions, informs physician promptly of significant changes in patient's condition, and provides written summary to physician within supplemental order.

Effectively and timely communicates with Spectrum Nurses' Clinical team, medical providers, patients and families/caregivers to facilitate continuity of care. Prepares clinical documentation in real-time during visits and submits to the nursing department to comply with established time lines for billing optimization. Completes all documentation legibly, and applies approved abbreviations and documentation error correction practices per Spectrum Nurses' policy.

Maximizes work efficiency through the use of computers and other automation technologies to validate plan of treatment orders, communicates patient care provided, and follows assigned visit schedule. Completes visit notes accurately and legibly according to agency submission requirements at the end of each work day.

Accepts accountability for own practice through ethical and professional conduct. Follows established programs and practice within policies and procedures reflective of Spectrum Nurses' mission, values and objectives.

Attends and completes required training modules, in-services, and continuing education to maintain competency and professional licensure for demonstrated knowledge regarding the care and management of patients in the home and/or alternate care settings.

Payment depends on the amount of patients one has/willing to take on and how often they are to see them per Doctor orders.

Payment is per visit:

Regular Visit: $50

Start of Care: $65
Re-certification of Care: $55
Resumption of Care: $60
