PSA: We got new reports that someone "from our office" called a patient and requested personal information in order to order a test for them. The patient wisely declined to provide any information to "John Wilson" who had a distinctly foreign accent.

No offense to foreign-born folks, but we do not have any working here answering the phone. So if you get a call supposedly from our office and you have a hard time understanding the person through their accent, it is NOT from us. We will not call you asking for personal details as we have all that info already. We also will not order a test out of the blue without personal consultation with the patient.

Unfortunately, technology allows the scammers to appear as whatever phone number they want to appear as. You might get a call from the Holmes County Sheriff's Office that is not from them also. Be alert, be vigilant, and always ask questions.

Cigna has announced this week that they will start downcoding physician notes automatically when they receive claims, starting in October. They did not disclose details of how they will decide which notes to downcode, but they did suggest that an AI would be involved. This will be done without reading medical records, to actually verify if the change is appropriate. This is unethical at least, and illegal in many states. They do allow for appeal of these decisions, but the process will be costly and lengthy to physicians that undertake it.

We believe firmly that this will ultimately lead to a lot of physicians, especially specialists, dropping Cigna insurance. If you or a loved one has this insurance carrier, please call them and ask for them to reconsider this inappropriate decision. We hope they will reconsider before they create a disaster for many patients.

To provide more information, all physician visits are basically a level 1, 2, 3, 4, or 5. The codes change based on the setting (hospital, ER, urgent care, outpatient), but that is the basic arrangement everywhere. The complexity of the case is determined by the provider/physician, and this is used to select the level applied. It is illegal for providers to upcode or downcode intentionally (either claiming more or less effort than was actually put into the visit). Historically, insurance companies would audit a record if they had reason to suspect it was upcoded by the provider. This move by Cigna announces these changes will take place without the appropriate audits. It should be illegal for them to downcode without actually checking the legitimacy of the change, since it is illegal to do this on the provider side.

We would like to take a moment to praise Sheriff Tate and the Holmes County Sheriff's Office. They have taken over management of Holmes County EMS; this has resulted in new workspaces being built and new equipment being purchased. This represents some major upgrades for the care our county receives from Holmes County EMS. It is a pleasure to see the agency being outfitted with new and better equipment.

Good news. Mary returns to us on Monday. The Contini's will be out of the office, but Filippo and Mary will be there. We are all grateful for the prayers lifted up for Mary and her baby!

When we move into our new facility on December 1, 2025, we will be adding a new doctor. Dr. Richard Moore, D.O., was raised in rural south Florida (back when there was such a thing). He will be joining us to assist with the growing population and needs in Holmes County and surrounding areas. One of his unique skills that he brings with him is osteopathic manipulation. Dr. Moore has been in practice in Jacksonville, FL, for 10 years but wanted to return to the country, where he grew up.

We apologize to our patients for longer wait times this week. Mary is unexpectedly out, and we do not know when she will return. She is ok, but would appreciate prayers for her situation. We will be adjusting our processes to accommodate for her absence next week and the week after, but this week has been longer than we want. Again, we apologize for the inconvenience of the situation and we are trying to remedy it going forward. Stay safe on these rainy roads.

Caring for the Whole Person

The Contini’s feel that values are extremely important. We moved to the rural setting to be closer to their patients, and to avoid the unnecessary hustle that healthcare is becoming in the big city. We moved to Bonifay to be close to family. We believe in treating the whole patient, not just biologically, but also emotionally and spiritually. Every patient’s personal values and family dynamics are a necessary part of their treatment plan. We do our best to incorporate these issues when making diagnoses and deciding on therapy. A new patient can expect to be listened to, and every attempt made to understand their perspective on their issues. Our patients are not just our patients, they are our coworkers, colleagues, neighbors, friends, and family. We strive to avoid unnecessary testing and expense for our patients, but we are not afraid to call on broader medical expertise when needed.
