Have I learned a lot? Yes.

Did I enjoy it? Absolutely not.😂

I would like to clarify - I have no problem with Medicare beneficiaries, however, I do have a problem with the way in which Medicare conducts business.

I’ve heard stories of clinics having to put caps on how many Medicare beneficiaries they see because Medicare refuses to pay them appropriately for the service services they provide. We’re limiting Medicare. Patient appointments to no more than 30 minutes, no matter the diagnosis.

I care deeply about the population of patients that I see, which is why I chose to take Medicare when everybody was telling me not to. That’s why I opted to only accept Medicare and cash pay. The time associated with the administrative tasks related to taking Medicare is so high I could not possibly think about taking other insurances. I would spend more time managing claims and denials that I would treating patients and that is not my priority.

Let’s talk mechanics⬇️

At heel strike, your foot is in supination (dorsiflexed and inverted) Thanks tibialis anterior!

Then it is (supposed to) roll into pronation for midstance to prep for toe off. Shout out to the windlass mechanism for making this possible!

I have had a number of patients who cannot dissociate these motions - that is, rolling from supination, to pronation, back to supination - due to weakness in their foot and ankle complex along with range of motion restrictions.

This presents as them walking on the outside of their foot throughout the entire stance phase and getting poor push off for swing.

The tibialis anterior controls pronation eccentrically, the tibialis posterior prevents the foot from overpronating and the fibularis/peroneal muscles concentrically pronate the foot.

These are some of the exercises I’ll send my patients home with to build strength in those eversion muscles so they can have more effective gait and have better stability/ balance.
