Get your medication refills in! We are open 8-5 tomorrow then closed on Thursday and Friday to enjoy our time with family and friends. One thing we are thankful for is our patients! We hope you cherish your time together with your loved ones as well.

Medicare Annual Enrollment started October 15 and ends December 13!

You can only update your Medicare plan once a year. Plans change each year, make sure your doctors, prescriptions and priorities are covered.

-Compare original Medicare, Medicare Advantage, Part D
-Ensure your preferred doctors, like those at CommunityFamilyPhysicians, are in-network
Call us during normal business hours to check our in-network coverage

It seems we have had some success with incoming calls. Try to call first and if you get a busy signal we continue to have portal and email access.

Attention patients: We are aware of a nationwide outage with our phone service. We are working on a temporary fix while it is being resolved. Phones will be forwarded to one of our front desk phones IF our VOIP service is able to do so.
