1️⃣ Arrival Rate
How many scheduled visits actually showed up?

If this is under 90–92%, your marketing isn’t the issue.
You’re leaking what you already have.

2️⃣ Revenue Per Visit
Total collected ÷ total visits.

If this number is low, more leads won’t fix it. You’ll just work harder for the same margin.

3️⃣ Next Week’s Schedule
How many open slots exist right now for next week?

If next week isn’t mostly full by Thursday, you don’t need better marketing.

You need better follow-up.

Here’s the hard truth:

Most clinics don’t have a marketing problem.

They have a conversion and system problem.

Marketing magnifies what already exists.

Marketing accelerates growth.

If they’re loose?
Marketing accelerates chaos.

When I talk to clinic owners I see this more than anything...

Owner treats 40+ hours.

Then goes home and runs payroll.

Answers staff texts.

Checks cancellations.

Worries about cash flow.

And tells themselves: “It’s just a season.”

But if that season has lasted three years… let me tell you, it’s NOT a season.

You didn’t open a clinic to feel trapped inside it.

You opened it for freedom... to spend more time with family... to build your dream home... travel the world... or whatever the vision for your life may be!!

But freedom doesn’t magically show up.

It comes from structure.

Numbers.
Processes.
Standards.

I used to think if the clinic was busy, we were winning.

We had a full schedule, the phones were ringing...

Of course, it felt good!

But then I’d look at the bank account and think…

“Why does this still feel tight?”...

Here’s what I finally understood:

Busy just means people are coming in.

Profitable means:

• Your revenue per visit makes sense
• Your arrival rate is tight
• Your therapists aren’t under-utilized
• You’re not leaking visits every week

Most clinic owners don’t have a marketing problem.

They have a visibility problem inside their own numbers.

And I get it. We didn’t go to PT school to stare at spreadsheets.

But once I started tracking the right metrics weekly, everything changed.

Can I be honest with you?

When I first opened my clinic, I didn’t track revenue per visit.

I tracked how tired I was.

I tracked how busy we felt.

I tracked how stressed payroll week made me...😖

And I kept telling myself: “If we just get more patients, this will fix itself.”

It didn’t.

I’m going to say something that might annoy a few people.

If your clinic isn’t profitable right now…

adding more patients probably won’t fix it.

I see it all the time.

Owner gets stressed.
Revenue feels tight.

So the instinct is:

“Let’s market harder.”
“Let’s run ads.”
“Let’s get busier.”

But here’s what actually happens:

More patients + same systems = More chaos, exhaustion and the same margin.

If your arrival rate is leaking…

Here’s the 4-step loop I use with clinic owners (and in my own clinics):

1) Track — weekly numbers (not monthly surprises)
2) Interpret — what’s moving + what’s leaking
3) Fix — one change this week (not 12 changes “someday”)
4) Repeat — consistency is the secret sauce

That’s it.
