Brief Job Description
A Resident Services Coordinator is responsible for assessing residents’ needs, coordinating care and services, and serving as a link between residents, families, staff, and outside providers. They help ensure residents receive appropriate support while maintaining independence and well-being.
Key Duties
Coordinate resident care plans with Administrator, nurses, caregivers, and physicians
Communicate with residents’ families about care and services
Schedule and supervise caregiving staff
Monitor residents’ well-being and address concerns or changes
Ensure compliance with state regulations and facility policies
