Proud to have sent our colleagues and to attend the Conference this November in National Harbor, Maryland. As we continue to listen and engage with complex, polychronic populations across the country, it is important to understand how we can better serve and deliver treatment and care to patients enrolled in Medicaid, SNP, and Medicare Advantage programs.

This year’s conference was focused on expanding access to care, and Monogram is proud to be at the forefront of this initiative as the leading in-home, multi-specialty provider for aging patients with multiple chronic conditions. Monogram’s model brings trusted clinicians directly to patients’ homes, including rural communities. November is National Diabetes Awareness Month, dedicated to raising awareness about diabetes, its prevention, and effective management.

To date, diabetes rates are on the rise and are often not a standalone condition. The urgency to act has never been greater. This fall, alongside the Diabetes Association, American Heart Association, and Jonas, Monogram supported the " Future of Diabetes " campaign to empower patients and providers with the latest strategies in prevention, treatment, and long-term management.

Monogram’s personalized approach to multiple-specialty, in-home treatment for patients with long-term chronic conditions is unique. Monogram Health bridges the gap between treating a condition and treating the whole person by evaluating and connecting the dots with other specialties to foster improved diabetes control and fewer complications.

Last week, Nashville Business Journal announced its “2025 Power 100: Disruptors” and selected Monogram Health’s Co-Founder and CEO, Michael Uchrin, as part of this distinguished group.

Monogram Health doctors Shaminder Gupta, Gorav Bohil, and Jordan Bley enjoyed their visit to the University of Pennsylvania's Leonard Davis Institute of Health Economics (LDI) with Rachel M. Werner and her team of senior fellows to learn firsthand the findings and insights garnered from the five studies that Monogram Health supported.

Monogram Health is proud to be at the forefront of clinical research focused on enhancing access to specialty providers, improving the uptake of evidence-based treatments, advancing the financing and delivery models of care for polychronic patients, and addressing racial inequities.

Penn LDI is the University’s hub of health services research with more than 500 Senior and Associate Fellows involved in interdisciplinary healthcare research, several of whom are focused on addressing barriers to evidence-based treatment.
