Most gaps in care aren’t caused by lack of effort. In hospice and home health, gaps in care are rarely the result of poor intent or clinical skill. More often, they are the predictable outcome of systems that were never designed to function across all hours.
When continuity depends on individual vigilance rather than structure, variability becomes inevitable. Issues resurface instead of resolving. Nurses absorb the burden of reconnecting the dots. Continuity isn’t something teams can improvise. It has to be designed.
