As a resident of Missouri, you and/or your family member may be eligible to receive Consumer Directed Services (CDS). CDS, is a program funded through the Missouri’s MoHealthNet Medicaid program to help adults with physical disabilities take control of their own care within their home, this may help prevent or delay hospital/nursing home admissions

To be be eligible for Consumer Directed Services the Participants must:
✔️Have active MoHealth Medicaid
✔️Be at least 18 years of age
✔️Have a physical disability
✔️Have the ability to self-direct your own care.

Consumer Directed Services (CDS) is a Medicaid-funded program that helps people with disabilities with their activities of daily living (ADL). This allows the consumer to live independently, prolonging residential care living. The CDS program allows the consumer to hire someone that they know and trust to provide their care.

Services includes:

* Bathing and showering
* Toileting
* Medication Reminders
* Transportation
* Meal preparation and cleanup
* Housekeeping
* Laundry
