The Connecticut Age Well Collaborative, a CCC initiative, is partnering with The Housing Collective to present 'No Place to Grow Old,' the first documentary to examine the growing crisis of older adult homelessness in the United States. Connecticut Community Care (CCC), a statewide nonprofit care management organization, helps more than 17,000 people of all ages and abilities to receive the care they need, at home, in their own communities, and offers health and wellness programs, corporate and community education initiatives. CCC reaffirmed its commitment to quality care management by achieving the highest level of Case Management Accreditation for Long Term Services and Supports (LTSS) for the Conecticut Home Care Program from the National Committee for Quality Assurance (NCQA). As the largest access agency for the publicly-funded Connecticut Home Care Program for Elders (CHCPE) and Adults with Disabilities (CHCPD), CCC serves individuals in northwest, north central and eastern Connecticut. Care Management Associates, CCC's private division, serves clients through private pay or long term care insurance.
