It’s official. Community Care, Inc. has joined the CareSource family of companies!

For more than 45 years, Community Care has helped seniors and adults with disabilities live as independently as possible in their communities. This affiliation with Ohio-based CareSource combines the strengths of our two organizations, allowing us to enhance innovation in health care, improve outcomes, and create a meaningful, positive impact. Together, we look forward to transforming health care for individuals with complex health needs in Wisconsin.

Community Care is a local nonprofit with more than 40 years of experience serving the needs of older adults and adults with disabilities. Our mission is to help our members live as independently as possible within the community.

We coordinate and deliver a full range of supportive services that help more than 12,000 Wisconsin residents live safely, confidently, and with dignity.

Our dedicated team works with members to determine their health and social needs and develops care plans to ensure those needs are met. We are proud to partner with more than 1,800 care providers and 15,000 individual practitioners at 6,000 locations.

Together, we’re committed to providing compassionate, personalized care that empowers members to live a healthy and independent lifestyle.
