2025 was a landmark year for Age-Friendly care at Maimonides.

Through our Advance Illness Management (AIM) and Age-Friendly Health System initiatives, we continued to elevate person-centered care for older adults—securing more than $1.1 million in grant funding, expanding caregiver support, and embedding meaningful conversations into care across the hospital, including the Emergency Department.

From new caregiver-focused programs to national recognition and thought leadership, this work reflects a shared commitment to dignity, respect, and care that centers what matters most.
