Plum Creek is so thankful for our residents, their families, our amazing team, and the many providers and community partners who help us ensure the best quality of life for everyone we serve. We’ve enjoyed many wonderful years filled with the best Thanksgiving fun — and we’re grateful for each moment. Our team shows up day after day — even on the holidays — taking time away from their own families to care for those who cannot care for themselves, and that dedication never goes unnoticed. We are truly honored to have the team we do, and we are equally honored to serve the very best residents.

It’s true that assisted living communities are not required to have a medical director — but we do, and ours is one of the best in the Medina and Greater Cleveland area. Dr. S.K. Jain, a highly respected physician with long-standing ties to Cleveland Clinic Medina Hospital, has been providing exceptional medical care to our residents since 2009. His Certified Nurse Practitioner, Andrew Black, CNP, is on-site at least once per week, ensuring consistent, high-quality senior healthcare.

Dr. Jain’s hospital privileges support seamless continuity of care, allowing residents to experience smoother transitions if hospitalization becomes necessary. Additionally, as the medical director for several skilled nursing and rehab facilities in Medina, he helps our residents maintain the same physician and NP through every level of care — assisted living, hospital care, and skilled rehabilitation- and to get them back home to Plum Creek as soon as possible!

This comprehensive, coordinated approach is one of the many reasons families choose our assisted living community for trusted, long-term senior care in Brunswick, Ohio.

We had such an honor hosting our friends from the Northern Medina County Chamber! 💙

One of the most special moments was hearing owner Kathy Elseser share just how truly family-owned and family-hearted Plum Creek is. She told us that her babies were baptized in the very chapel our guests were sitting in — because she wanted the residents (who she considers family) to be part of that moment. 🥹💛

It’s stories like these that show what makes Plum Creek so special. Thank you to the Chamber for spending the day with us!

We are so grateful for our wonderful partnership with St. Ambrose Catholic Parish, a relationship that has blessed us since 2009! From the very beginning, they have generously provided services in our beautiful chapel here at Plum Creek, and we are truly thankful for their continued support.

This week, we had an amazing time visiting St. Ambrose—sharing lunch, handing out prizes for their game, and most importantly, spending time with all of the seniors. We loved the opportunity to connect, laugh, and share more about everything happening at Plum Creek.

Here’s to many more years of friendship and community! 💛🌟

Plum Creek Assisted Living was truly honored to take part in this event at The Villas at Bakers Ridge celebrating those who give so much to our community. ❤️

Social workers, nurses, and case managers from Cleveland Clinic, UH, Southwest Hospital, local nursing homes, and more joined us for dinner (and some very yummy cookies), a CEU presentation, and a festive Christmas-wreath-making workshop—complete with delicious cocktails! 🎄🍸✨

Owner & VP of Operations Kathy Elseser helped work the event, and our very own Debby Rea made the most amazing cookies—seriously, everyone was talking about them! 🍪

It was the perfect way to kick off the holiday season, and we are so grateful for the opportunity to support and partner with such dedicated professionals.

A Thanksgiving Message from Our Team

As Thanksgiving approaches, we want to take a moment to express how truly thankful we are for our residents, their families, our dedicated team, and the wonderful community that surrounds us. Your support and partnership mean so much, and we’re grateful to share this season of gratitude with you.

Thanksgiving Meal – Wednesday, November 26th at Noon
Family members, please remember to RSVP for our Thanksgiving meal by Friday, November 14th. Your RSVP helps us create a thoughtful seating chart—because we never want any resident sitting alone on such a special day.

Holiday Outings
If your loved one will be going out for Thanksgiving, please let us know the details ahead of time, including:
• Who will be picking them up
• When they’ll be leaving
• When they’ll be returning

This information allows us to:
a) Keep everyone accounted for and safe
b) Prepare medications ahead of time (which is especially helpful during busy holiday schedules)
c) Ensure your loved one is ready when you arrive
d) Plan meals accordingly
