🚨New Patient Portal: Follow My Health🚨

For those who may not know, we are now using a new patient portal called Follow My Health. This portal is available for our patients 𝟣𝟪 𝗒𝖾𝖺𝗋𝗌 𝖺𝗇𝖽 𝗈𝗅𝖽𝖾𝗋, and we’re happy to assist anyone who needs help signing up.

▪️𝐇𝐨𝐰 𝐭𝐨 𝐆𝐞𝐭 𝐒𝐭𝐚𝐫𝐭𝐞𝐝▪️

Step 1: Receive Your Invitation Email
You will receive an invitation email from our office to create your Follow My Health account.
• Please make sure we have your correct email address on file.
• If you do not receive the invitation, contact our office for assistance.
• The email subject line will be from: FOLLOW MY HEALTH

Step 2: Create Your Account
Click the link in the invitation email and follow the prompts.
When asked for an invitation code, enter your full date of birth in the following format:
MMDDYYYY (no spaces, dashes, or hyphens).

Step 3 & 4: Accept Agreements

Step 5: Download the App
At the final step, you will be prompted to download the Follow My Health app.
• Click the App Store link to complete the free download.

Step 6: Log In
Once the app is downloaded:
• Use the email address where you received the invitation link as your username.
• Enter the password you created during setup.

We are aware that patients are receiving notifications that lab results are available but are unable to view them. This is because our providers do not release lab results until they have personally reviewed them. Once reviewed, the results will be released and available for you to view in the patient portal.

We are also aware that when you send a message to a provider through the portal, it may no longer appear in your message history. Please be assured that your message has been successfully sent, and there is no need to resend the same message. Repeated messages can create duplicates and slow response times for our providers.
