Navigating the Way: 4 Steps to Your Prenatal Care and Family Leave!

Here's what you may need during pregnancy:
1. Prenatal Care Medical Certification. Use this form when you need medical leave to attend your prenatal care
2. Prenatal Incapacity due to a prenatal health condition. Use this form if a health condition comes up during pregnancy that requires you to attend extra appointments with your midwife, OB, or specialist OR if you need to stop working
3. Certification of Birth. Use this form to announce to the state and to work baby has been born (you get this when you are discharged from the birth center or hospital but be sure to ask for it!) This activates the Parental Leave. Calculate your estimated coverage here: paidleave.wa.gov/estimate-your-weekly-pay
4. Medical Certification for Birth Complications. This form is used to let the state and your work know that you have a medical condition (including a recovery from surgical birth), or any medical condition postpartum that requires extra treatment or recovery.

At Rainier Valley we have these signed and ready to complete for you in your patient portal. At other locations, ask your midwife or OB to sign them for you (bring them printed).
