Many thanks to my panel co-moderator and speaker Dr. Zakia Rahman for helping me to put together our third ASDS FutureTech session. This year’s theme was “Seeing Beyond our Eyes.” We had terrific talks by Dr. Rahman as well as Dr. Sherrif Ibrahim and Dr. Babar Rao (not pictured).

Dermatology is rapidly evolving in laser and energy based devices, noninvasive imaging, and augmented intelligence. It’s great to stay up to date!

(I had to wear my Iron Surgeon costume to moderate the panel… the schedule is so busy I didn’t have time to change!)

Real talk from Dr. Susan Weinkle ! Whether I’m doing Mohs surgery anesthesia or cosmetic injections, I aim to be slow and gentle with those needles. You can never prevent bruising 100% but I look out for those superficial vessels and my experience with the deeper anatomy allows me to avoid the major danger zones for bruising or bleeding.

I was honored to speak on behalf of Regeneron and present the data supporting the new FDA Indication for Libtayo (cemiplimab). This immunotherapy was the first approved for locally advanced or metastatic cutaneous squamous cell carcinoma, and now it has a new indication: as an adjuvant therapy for cSCC at high risk of recurrence after surgery and radiation. Treating patients with cemiplimab after their surgery and radiation enabled a 68% benefit in disease-free survival. Fortunately, the majority of cSCC can be treated by surgery, but for the few that need radiation and are at high risk, it is reassuring to have an additional option.

I am a paid speaker for Regeneron and a consultant on skin cancer research - this is what we call a COI or conflict of interest. It means that I have the potential for a conflict of interest in recommending this treatment for patients. I do my best to mitigate this issue by being open about disclosing this relationship and by referring to the radiation and medical oncologists that will ultimately make these decisions with our patients.

Tranexamic acid is a game changer for melasma and hyperpigmentation, but it does come with risks. TXA promotes blood clots, which is helpful for surgery or women with heavy periods - but the down side is it increases clotting risk in everyone else too. The low doses we use in dermatology are typically low risk for clots, but many women are on other clot-promoting drugs, especially the pill for contraception or estrogen for hormone replacement. Doctors need to be very careful about combining two agents. Short term use of TXA may not be as risky as long term use, but in some cases it’s not possible for patients to be on oral TXA. Talk with your dermatologist about hormone contraception or estrogen therapy if you are considering oral TXA for pigmentation. Fortunately, alternative topicals exist so don’t lose hope!

I had a total fangirl moment running into the incomparable Mohs & reconstructive surgeon Dr. Tri Nguyen at a hands-on cadaver surgery course. Dr. Nguyen’s lectures have taught me so much over the years and I was inspired to see he was taking the class as a student, not a teacher! It reminds me that we can always learn more, no matter how experienced we become.

The class was focused on the surgical anatomy for blepharoplasty (eyelid lift), lip lift, brow fixation, and face lifting. It was such a gift to be able to do a facial dissection and look at all the anatomic features in detail. I am inspired to do better for my patients! (Note: I’m still not offering deep plane facelifts.)

Great talk from .nahidvidal on strategies to work with anxious patients. I do my best to help my patients feel calm and confident before surgery, whether it is for skin cancer or cosmetics. I appreciate new strategies to add to my communication skills.

It was so much fun catching up with these brilliant women at the American Dermatologic Association conference last week. We all trained at UCSF in overlapping years and even though we’ve drifted apart in space and time, we can come back together to laugh and reminisce as if no time has passed. From L to R, Dr. Shinkai is now a Professor at UCSF and Editor in Chief of JAMA Dermatology, Dr. Maguinness is a Professor of Pediatric Dermatology at U of Minnesota, and Dr. Gilliam is a pediatric dermatologist with me at PAMF/Sutter Health! How lucky I am to have these women as colleagues and supporters!

I love when patients appreciate the precision of Mohs micrographic surgery. With this procedure, we are able to remove skin cancer in fine layers and evaluate the entire tissue margin under the microscope in real time. I track cancer cells until the entire area is free of disease. Then I work with my patients to design the optimal reconstruction for their wound. The worse part of the day is the injection of local anesthesia, but we use techniques to ease that pinch and sting as much as we can. Nobody enjoys having skin cancer or skin cancer surgery, but we work to make the entire experience as comfortable as possible for our patients.
