Can’t afford your medications? You may qualify for our Prescription Assistance Program.

Are you eligible?

 You must use a local physician.
 You must be taking long-term medications prescribed by your physician at his/her local clinic.
 You must meet the financial guidelines set by each pharmaceutical company from which you are requesting medications.

If you feel that you are eligible for this program then stop by our office to fill out the application. You will need the following documents:

 A copy of last year’s Income Tax Return, W2 or Social Security Benefits Statement.
 Picture I.D.
 List of prescribed medicines and dosage information. (Please bring actual prescription bottles)
 Copy of Medicare, Medicare Part D and/or Medicaid card.
 Copy of Social Security Card

There is a charge of $15.00 for each 3 month supply of each prescription ordered (and reordered). This fee should be paid when you pick up your prescriptions. The Christian Health Center is a non-profit organization and operates solely by donations from churches, business and individuals. We appreciate your support of the administrative costs in providing and operating this program.

We help people apply for these programs but the drug manufacturers decide who qualifies.

Office hours are 8:30AM to 4:30PM Monday through Thursday. Closed for lunch 12-1.

Remembering Sylvia Bevill this morning. I found her business card cleaning out some old papers. She was a volunteer nurse at our clinic for a lot of years. Such a sweet Christian lady. We miss you Sylvia.

The Christian Health Center / The Hub will be closed tomorrow ( Wednesday, July 9) 9:15 am- 11:45am due to a funeral. We will resume normal business hours for the remainder of the day. Thanks for understanding.
