Due to circumstances beyond our control; we are announcing the closure of TLC Family Clinic effective immediately. It has been a great pleasure servicing your health care needs. We would like to thank you for choosing TLC Family Clinic and if you have any questions or need assistance please call the office at 575-885-9199.
At this time, you will need to find another provider for your healthcare needs, we can assist you in this process if needed. When you find a new provider, you will need to complete an authorization to release your medical records so that we can forward a copy of your records to your new physician. Unfortunately, your medical records cannot be released without a written authorization from you. This can also be done by the new provider you have selected. They can fax the request to 575-628-0029. If you would like a copy of your medical records given to you; you can come by TLC Home Health Care, LLC located at 320 W Mermod St.
