I am a dedicated nurse with years of experience working in the senior care industry . I have experience in all facets of geriatric health including acute, long-term, home, and hospice care. I provide direct assistance to seniors by coordinating and managing care both in their homes and in care facilities. I partner with licensed social workers, geriatric physicians, physical therapists, home care providers, and others who work in the senior care field. My goal is to create close relationships with our senior clients and their families that provide skilled, knowledgeable and caring support. Some of the services that I provide at Aging Intuitively include:

• Health advocacy and care management. Coordinate care and acts as a liaison between client, family, and health care providers ensuring continuity of care
• In-depth assessments and custom service plans
• Accompany clients to medical appointments and outpatient procedures
• Coordinate and supervise home health and caregiving teams
• Facilitate discharges from hospital and skilled nursing facilities
• Wellbeing checks and status calls to client’s loved ones
• Home safety evaluations
• Serve as local emergency contact and connection if family do not live nearby or are out of town on vacation
• Assistance with Medicare issues and long-term care insurance filing
• Elder care resources and referrals including placement assistance
• End-of-life consultations, planning, care coordination and support
• Eldercare consultation packages
• Caregiver coaching
• Senior companion and engagement
