The team at A Hand To Hold is made up of dedicated professionals who specialize in senior care. We have experience in all facets of geriatric health including acute, long term, home and hospice care. A Hand To Hold provides direct assistance to seniors by coordinating and managing care both in their homes and in care facilities. We partner with licensed social workers, geriatric physicians, physical therapists, home care providers and others who work in the senior care field. Our goal is to create close relationships with our senior clients and their families that provide skilled, knowledgeable and caring support. Some of the services that we provide at A Hand To Hold, Senior Care Coordination include: • Health advocacy and care management.
