Following the recommendations of the American Academy of Pediatrics and the American Academy of Allergy, Asthma & Immunology, we will now be offering in-office oral challenges for many patients with documented penicillin/amoxicillin/Augmentin allergies!

Per the American Academy of Allergy, Asthma and Immunology:

“Approximately 10% of patients report an allergy to penicillin. However, the majority of patients (greater than 90%) may not truly be allergic. Most people lose their penicillin allergy over time, even patients with a history of severe reaction such as anaphylaxis.”

We now offer a safe, supervised, in-office oral penicillin challenge using a two-step method:

1. Your child will be given a very small amount of penicillin and observed for any signs of reaction.
2. Your child will then be given a larger dose of penicillin and observed again.

If no reaction occurs, the penicillin allergy can be removed from your child’s medical record, making it easier to treat future infections with the most effective and infection-specific antibiotics.

The full appointment will last just over an hour.

*Patients with a history of true anaphylaxis will not be eligible for this test.

Flu still raging:

If your child is *exahusted* with zero appetite and some fevers, it’s probably Flu A.

-Lots of high fevers (there’s no magic degree cutoff where the fever is “more serious”), acetaminophen and ibupforen can help with discomfort with fevers

-Breathing always looks worse when they have a fever

-Appetite will take a while to come back. Try popsicles to get some fluid in them.

Reasons to call/come in: you’re worried about dehydration (not able to keep any fluids down for a few days, decreased or dark urine), breathing concerns, changes in mental status, not starting to improve after 4-5 days.

It’s still not too late to get your flu shot! The kids I’ve seen who are the most ill have not had a flu shot.

🎄 Holiday Schedule at Carmel Pediatrics 🎄
On December 24, 26, and 31, we will be open for a few hours in the morning for urgent sick visits only.

📞 Phones turn on at 8:00 AM and will likely turn off by 10:00 AM, so please call early if your child needs an urgent appointment.

🚫 We will be closed on December 25 and January 1

Shout out to Nurse Amy for manning the lit-up phones today- here’s her advice!

Friendly PSA from Nurse Amy:

Influenza A is RAMPANT right now. Symptoms can include all or some of the following:

Fever 🤒
Sore throat 😫
Body aches 🫩
Chills 🥶
Headache 🤕
Nausea/vomiting/diarrhea 🤢
Fatigue 😴
Cough 😮‍💨
Congestion 🤧
Dizziness 🫨

If you have these symptoms, stay home and take care of yourself until you are fever free for 24 hours and symptoms are improving. Call your doctor or go to the ER if you are having difficulty breathing or dehydrated.
If you do not want these symptoms, wash your hands frequently, do not touch your head holes, stay away from sick people and if that’s not possible, wear a mask. 😷

Flu A (and a few Flu B) is hitting hard.

Lots of fever, sore throat, fatigue, body aches. The kids who got their flu shots seem to be less ill (it’s not too late to get one, but do it now!).

We don’t love Tamiflu for kids who aren’t high risk (can cause hallucinations, mental status change, nausea/vomiting).

Otherwise, supportive care measures/pain control with acetaminophen or ibuprofen (ages 6 months and over). Honey can help with cough, but we don’t recommend any other cough or cold medicines.

No need to be seen in the office unless you’re worried about breathing, hydration, severe muscle pain, or other concerns.

Stay safe out there!
