Chuck Combs passed from his earthly body into the presence of his Lord and Savior Jesus Christ on August 18, 2021 after a short but fierce battle with cancer.
Carmel Rehab will be closing immediately. Please seek the services of another physical therapy provider to continue your care. The physician that referred you for treatment might be able to recommend specific alternatives. Many health insurance companies include a Provider Search function on their web site that could provide you with a list of options that are in-network with your specific insurance plan.
A custodian of medical records has been appointed to ensure that your interests will be protected. We can make arrangements to transfer a copy of your records to a new provider if they contact Carmel Rehab directly. If you would like to request a personal copy of your medical record, please let us know. The fee for this service is $20. You may contact Carmel Rehab by phone at (317) 818-0404 through mid-November. After that time please send a written request to PO BOX 3566 CARMEL, IN 46082.
We are so thankful for the 23 years that God allowed Chuck to practice physical therapy at Carmel Rehab, and for the role you played in his life.
