Shandi Duff, CCMH Community Health Worker, will be available March 17 from 1:30 p.m. – 3:30 p.m. at the Carroll County Health Department (5 N Ely Street, Carrollton) to connect individuals and families with valuable resources.

A Community Health Worker (CHW) is a frontline public health professional who serves as a vital link between healthcare services and the communities they serve — promoting health equity and improving access to care.

During this visit, Shandi can assist with:
• Uninsured or underinsured support
• Medicaid enrollment
• Marketplace insurance
• SNAP benefits
• Additional community resources

All services are free of charge.
